AB70. Clinicopathologic analysis of alpha fetoprotein-producing gastric carcinoma

Objective: To investigate the development of alpha fetoprotein-producing gastric carcinoma (AFPGC) and the relationships between alpha fetoprotein (AFP) and gastric carcinoma.

Methods: To review and summarize domestic and foreign literatures about AFPGC as well as retrospective analysis and follow-up clinical data of 210 AFPGC patients from 2,268 gastric cancer (GC) patients, from March 2009 to March 2012.

Results: AFP is of important reference value to the diagnosis, pathological type and prognosis of the AFPGC as a specific tumor marker. The proportion of GC patients with abnormal serum AFP level was 9.26% (210/2268), including 134 males and 76 females. Most of AFP was expressed at a low leve1. The percentage of cases with serum AFP level ranging between 10-50 mg/mL and 50-100 mg/mL were 67.1% (141/210) and 4.8% (10/210), while between 100-1,000 mg/mL were 20.9% (44/210), above l,000 mg/mL were 7.1% (15/210). Poorly differentiated adenocarcinoma were the most common clinicopathologic type with a percentage of 49.0% (103/210). Lesion located at antrum or angle of stomach were 52.4% (110/210) while 30.5% (64/210) at whole stomach. Patients in clinical stage III and IV accounted for 28.1% (59/210) and 66.7% (140/210). Only 67 cases (31.9%) were received radical gastrectomy. The mortality rate of 1 year, 3 and 5 years was 107 (51.0%), 25 (11.9%) and 33 (15.7%) respectively. There were 25 (11.9%) patients who have survived more than 3 years.

Conclusions: AFPGC is a special type of gastric cancer with higher grade malignancy and poorer prognosis. It still needs further study on the mechanism and treatment for AFPGC.
