Objective: The purpose of this study was to compare the accuracy of double contrast enhanced ultrasound (DCUS) with spiral CT in the preoperative judging lymph node metastasis for different parts gastric malignancies.

Methods: This study included 180 patients with biopsy proven gastric cancer who underwent surgical resection as primary management of their malignancies. All patients underwent DCUS and spiral CT prior to surgical intervention, the results of the ultrasound and CT findings compared with the pathological results of the resected lymph nodes and compared with each other too.

Results: The overall accuracy of DCUS and spiral CT was 86.0% vs. 83.9% in gastric antrum and pyloric carcinoma, there was no significant difference between them (P>0.05), and so did in the patients whose tumors located in stomach body or all over the stomach, the results of them was 82.5% vs. 85.0% respectively. But the overall accuracy of DCUS was lower than spiral CT in the cardia or fundus gastric cancinoma (66.0% vs. 85.1%, χ2=4.663, P=0.031).

Conclusions: Double contrast-enhanced ultrasonography offers a high overall accuracy in the preoperative lymph node evaluation of gastric cancer, which is similar to spiral CT. But its overall accuracy was quite low in the cardia or fundus gastric cancinoma.

Keywords: Stomach neoplasms; contrast-enhanced ultrasound (DCUS); spiral CT
