AB52. Remnant stomach modified roux-en-Y esophagojejunostomy in the treatment of esophagogastric junction cancer

Objective: To investigate the optimal reconstruction technique after proximal gastrectomy in the treatment of esophagogastric junction cancer.

Methods: A total of 110 patients with esophagogastric junction cancer who underwent proximal gastrectomy in Sichuan Cancer Hospital between January 2010 and December 2011 were divided into 3 groups according to the reconstruction technique: group A (Remnant stomach modified Roux-en-Y esophagojejunostomy, n=37), group B (Roux-en-Y esophagojejunostomy, n=37), group C (esophagogastrostomy, n=36). Operation time, quality of life (QOL), nutritional status 1 year after surgery, and perioperative complications were analyzed.

Results: There were no significant differences in Operation time and perioperative complications (P>0.05). One year after operation, QOL (Visick index) was better in group A than that in group B and C (P<0.05).The increased weight gain, in food intake, albumin were better in group A than those of group B and C (P<0.05) .The prognostic nutrition index ratio of group A was better than that in group B and C. The incidences of dumping syndrome, reflux esophagitis in group A were significantly lower than those in group B and C (P<0.05).

Conclusions: Remnant stomach modified Roux-en-Y esophagojejunostomy which prevents the reflux esophagitis and dumping syndrome, preserves the pathway of duodenum and promotes, is associated with improved nutritional condition and quality of life. It is a reasonable reconstruction method after proximal gastrectomy.
